NEW PATIENT PROCESS
We are very excited that you have chosen us for your child's pediatric care. Below are the steps that need to be taken in order for the office to schedule an appointment for your child/children.
***IMPORTANT NOTICE***
As of September 17, we are only accepting newborns as new patients
Please read the following important information about the Practice:
Preferred Pediatrics is a practice that integrates Holistic Care into our
appointments. We are NOT Naturopathic doctors. Because we
are a Primary Care facility that offers alternative care we spend more
time with each patient but book at a Primary care office pace.....this
translates into possible wait times. Please plan accordingly and
be patient and courteous with us.
We charge for Well Visit and Sick visit no shows.
REGISTRATION FORMS:
Either Print out (or download on to CD) the following forms:

Patient Registration Form
2.
Preferred Pediatrics Financial Policy
3.
Acknowledgment of Receipt of Notice of Privacy Practices
4. Bring or mail the completed forms (or CD) to the office:

MEDICAL RECORDS:
Please contact your previous pediatrician and ask them to MAIL the medical records to our office, or you can drop them off in person.
Please note: We will accept hard copies of your records only.

- WE DO NOT ACCEPT ELECTRONIC RECORDS via email)
Please allow the office up to one month to process all of the above paperwork. We CANNOT schedule an appointment until the registration and records are entered into our system. Once your registration is completed you will receive a link from the office asking you to join the patient portal. After you complete the portal registration you can call the office to schedule an appointment.
